Patient Rights: Services

You have the right to:

  • Considerate and compassionate care .

  • Respect for your personal values, beliefs and difference.

  • Professional and compassionate response to patient requests made for service.

  • Receive care in a safe setting, free from mental, physical, sexual or verbal abuse and neglect, exploitation or harassment. 

  • Access protective services and advocacy services including notifying government agencies of neglect or abuse.

  • Exercise these rights without regard to sex, race, color, religion, ancestry, national origin, age, disability, medical condition, marital status, sexual orientation, gender identify, gender expression, educational background, economic status or the source of payment for care. Cognitive & Behavioral Health Services does not exclude, deny benefits to or otherwise discriminate against any person on these bases in admission to, participation in or receipt of the services and benefits under any of its services, whether carried out by Cognitive & Behavioral Health Services directly or through a Community Resource or Social Programs, which Cognitive & Behavioral Health Services arranges to carry out its programs and activities. Inquiries, grievances or complaints related to this nondiscrimination policy may be directed to our headquarters by contacting 415-894-5581.

Patient Rights: Information

You have the right to:

  • Know the name of the Therapist and Clinical Care Manager who has primary responsibility for coordinating your care, as well as the names and professional relationships of other medical specialist and non-physicians who will see you to provide comprehensive care.

  • Receive information about your health status, diagnosis, course of treatment, prospects for recovery and outcomes of care (including unanticipated outcomes) in terms you can understand. 

  • Effective communication and to participate in the development and implementation of your plan of care.

  • Participate in ethical questions that arise in the course of your care, including issues of conflict resolution, withholding resuscitative services, and forgoing or withdrawing life-sustaining treatment.

  • Refuse to participate in research projects.

  • Access information contained in your records within a reasonable time frame, except in certain circumstances specified by law.

  • Reasonable continuity of care and to know, in advance, the time and location of appointments as well as the identity of the persons providing the care.

    Your Patient Rights: Privacy

  • Have personal privacy respected. Case discussion, consultation, examination and treatment are confidential and should be conducted discreetly. 

  • Be told the reason for the presence of any individual. 

  • Confidential treatment of all communications and records pertaining to your care with Cognitive & Behavioral Health Services. You will receive a separate "Notice of Privacy Practices" that explains your rights in detail and how we may use and disclose your protected health information.

As part of your daily care, you can expect to:

  • be given thoughtful and respectful care without discrimination of any kind.

  • be treated with respect for your cultural, mental and spiritual needs and choices.

  • be cared for in a safe setting, free from mental, bodily, or verbal abuse or neglect.

  • make choices about your care and be able to say “no” or ask for medically appropriate and necessary treatment.

  • be introduced to your care team and  have your plan of care explained to you in simple language.

  • be able to change your mind about the course of treatment.

As our partner in care, we expect to:

  • always act in a safe and responsible way and to know that physical violence or threats of hurting people will not be allowed.

  • never use rude, intimidating, derogatory or sexually inappropriate comments.

  • never behave in a disruptive manner, such as using loud or offensive speech, cursing, or verbally abusing our staff, guests and volunteers. 

  • cooperate with your treatment team when developing a treatment plan. 

  • respect Cognitive & Behavioral Health Services property, policies, rules and regulations.
